Job Description:
A company is looking for a LPN Case Manager to join their team
in New York, working remotely.
Key Responsibilities
Perform case management and coordination of care within an
interdisciplinary team
Collaborate with healthcare partners on complex patient care
coordination
Provide health education, coaching, and treatment decision support
for members
Required Qualifications
Current unrestricted NY LVN license
2+ years of experience in a physician's office, clinical, or
hospital setting
Knowledge of chronic diseases, especially COPD/asthma, diabetes,
CHF, and IHD
Proficient computer skills to work efficiently with electronic
medical records
